THIS FORM IS INTENDED FOR FAMILIES WHOSE CHILDREN ARE NOT AT MOSAIC SCHOOL. IF YOUR CHILD IS CURRENTLY ATTENDING MOSAIC SCHOOL, A SIMPLIFIED ENROLMENT FORM IS AVAILABLE ON THE PARENT PORTAL. A DIRECT LINK IS AVAILABLE AT THE TOP OF THE PAGE.
